Fees and Insurance: If you are paying out-of-pocket, please talk to me about my fees. If you are using insurance, please provide me with the information at the 1st session including co-pay amount, deductible, and authorization number (if required), Co-pays and charges related to unmet deductibles are expected to be paid at each session. My billing service will complete the paperwork and submit insurance claims. I take most insurance, including United, Cigna, Anthem Blue Shield Blue Cross, Aetna, Cofinity.
Upon Arrival: Please arrive 5-10 minutes before your scheduled appointment to complete paperwork. Take the stairs to the 3rd floor waiting room. I will greet you in the waiting room at your appointed time.
